Registration
If you are a new patient, we will ask that you complete New Patient information forms in our office. These forms provide valuable information for the physician and will enable us to establish a history and administrative file for you. If you are an established patient and information concerning your insurance, home address, phone number, or place of employment has changed, please let us know so that we may update our records accordingly. It is the patient's responsibility to notify us of any personal record changes. New patients should also bring the following items to the first appointment:
- Photo ID
- Insurance cards
- Any previous X-rays, MRI films and reports, or CT films and reports (The actual films will be needed; the reports alone are not sufficient.)
- Operative reports (if you have previously had surgery)
- List of any medications you are currently taking
- Motor vehicle accident police report (if your injuries were sustained during an automobile accident)
- Copayments or deductibles
